Order supplies

This form is to be used by MERI Outreach Committee members to order supplies for our outreach programs. This includes supplies for bovine eye dissections, copies of handouts and other items for our kits, and additional small items that we give out to children at our event (e.g., bouncy eyeballs). With regard to the handouts, please note that all handouts are available through the website and you can preview/download each handout from within this order form. Thus, feel free to download and print out your own handouts if you only need a small number or will just be using them for your own education (e.g., as might be the case with kit instruction manuals)
